L’annonce d’une mauvaise nouvelle est l’une des tâches quotidiennes du médecin hospitalier et demande des capacités communicationnelles élevées (Chumpitazi, C. E.et al., 2016). Dans une discipline comme la médecine où les compétences techniques sont essentielles, l’enseignement de techniques communicationnelles n’est pas la priorité (Lamba,Tyrie, Bryczkowski, Nagurka, 2016). Pourtant, selon Contro, Larson et Scofield (2002), l’annonce d’une mauvaise nouvelle est fréquemment délivrée de manière inadéquate et est très souvent source d’anxiété pour les professionnels de la santé et pour le patient. Ce mémoire se penche sur l’impact d’une formation courte basée sur la simulation par jeux de rôles sur la manière d’annoncer une mauvaise nouvelle au sein des Urgences, en la comparant à l’enseignement traditionnel, le stage. L’objectif est de comparer cet impact en termes d’évolution des compétences communicationnelles, du sentiment de compétence et d’une diminution du stress ressenti par le médecin lors de l’annonce de mauvaise nouvelle simulée au sein du département des Urgences, et de montrer une amélioration plus importante des compétences communicationnelles des jeunes médecins sur le terrain lors de consultations tout-venant aux Urgences. Pour ce faire, un échantillon de 35 médecins stagiaires a été constitué, dont 6 d’entre eux ont été suivis sur le terrain lors d’une consultation. Les participants ont été divisés aléatoirement en deux groupes : un groupe expérimental qui a reçu la formation entre les deux phases de test et un groupe contrôle qui a reçu la formation après les deux phases de test. Les phases de test étaient constituées d’une entrevue avec deux acteurs à qui les participants devaient annoncer une mauvaise nouvelle. En outre, six entretiens ont été menés avec des médecins urgentistes dans le but de modifier ou de valider le protocole SPIKES enseigné lors de la formation. Globalement, la formation a permis aux participants d’acquérir une structure pour annoncer une mauvaise nouvelle lors d’une consultation simulée et d’augmenter leurs compétences communicationnelles de façon significatives. Par contre, les résultats sur le terrain sont peu significatifs, peut-être à cause de l’échantillon restreint ( n = 6). Enfin, des entretiens avec des médecins urgentistes ont permis de valider le contenu de la formation donnée aux participants.

Dat is een situatie dieslechts weinig artsen geduldig aanvaarden. Zieke dokters ontkennen vaak hun ziekte, en gaan eerst zelf dokteren. Pas als dit niet helpt zoeken ze professionele hulp, en dan meestal bij een bevriende specialist. Maar dit betekent niet zomaar dat onderzoek en behandeling dan normaal verlopen...
